"Replace" Module broken?

I installed the Replace module and when I try to replace an image with it (it was a .png in this attempt), the resulting dialog just shows this (as raw code):

Is this happening to anyone else? It seems the dev of the module abandoned it so I'm wondering if a recent G3 update broke something.
