Is it possible to setup a carbon theme album so that it opens on the first item (with navigable thumbs) like in a slider theme, rather than showing an intermediate page of items' thumbs? For viewing consistency, I'd avoid using the slider.
I imagine a simple modification the carbon theme to achieve this effect would also be possible?

I don't know about simple. But I can't see why it couldn't be done. Though I'm not that familiar with either theme so I'm not sure where I'd start.
So you want the album view to show a larger size of the first image in the album with a row of thumbs below?
A few things I'd take a look at:
-- The new slideshow in 2.3 (click the link to my site below, go to any album and click on Slideshow to check it out.
-- The Ajaxian theme http://codex.gallery2.org/Gallery2:Themes:ajaxian
Demo that theme and many others here: http://themes.gallery2.hu/index.php?g2_theme=ajaxian&g2_colorpack=none&g2_frame=none

Thanks, I can see that ajaxian would do this but the visuals are quite different. However, I've just done a bit experimenting with the X_treme theme and it does just what I need. It has an option to open albums either into their thumbs or to the first item with the thumbnails for navigation.
It looks like a huge learning curve to use the X_treme theme but I am certain that it can be configured to almost mirror to what I already have in my version of the carbon style sheet.
